SMLE Question Bank for Pakistani Doctors: How to Choose and Use One
There are no official SMLE past papers, so a good question bank is the core of your prep. Here is what to look for, and how to use one against the SMLE blueprint.

Search for SMLE preparation material and you will find recalled “SMLE questions PDF” files, Telegram groups and paid platforms, all claiming to be the closest thing to the real paper. Since SCFHS does not publish past papers, the quality of your question bank matters more here than for almost any other exam. This guide covers how to judge a bank and how to use it once you have one.
Why a Question Bank Matters for the SMLE
The SMLE is a 300-question computer-based exam that tests applied clinical reasoning through patient scenarios, not isolated facts. Weighted roughly as Internal Medicine 30%, Obstetrics & Gynaecology 25%, Paediatrics 25% and Surgery 20%, it rewards candidates who have practised hundreds of vignettes across all four areas. Because SCFHS draws questions from a continuously refreshed item bank, there is no fixed paper to memorise. We explain this in SMLE past papers: what actually exists and what works.
What to Look for in an SMLE Question Bank
- Clinical vignette format. Questions should present a patient and ask for the diagnosis, the next step or the management. Plain recall lists do not match the exam.
- Blueprint-aware coverage. Medicine, Ob/Gyn and Paediatrics make up about 80% of the exam. A bank that is mostly one specialty will leave gaps.
- Explanations for every question. The reasoning is what transfers to a differently worded question on test day. A bare answer key does not teach that.
- Subject-wise practice and analytics. You need to filter by specialty and see where your accuracy is lowest.
- Timed mode. Pacing 300 questions is a skill. Look for a mode with a running clock and no answer reveal until the end.
- A free sample. You should be able to judge the questions and explanations before paying.
Be Careful With Recalled Question Files
Recalled sets go stale as the blueprint and item bank change, they are often inaccurate, and they rarely include explanations. SCFHS also prohibits distributing exam content. Original questions written to the SMLE style and blueprint are a safer foundation.
How to Use a Question Bank Over 8 to 12 Weeks
- Weeks 1 to 4: work subject by subject in untimed (tutor) mode and read every explanation, including for the questions you got right.
- Weeks 5 to 8: weight your question volume by the blueprint, with the bulk in Medicine, Ob/Gyn and Paediatrics, and revisit your wrong answers a few days later.
