Every Pakistani MBBS graduate eventually faces a version of the same question: stay and practise in Pakistan, or license abroad — and if abroad, where? Four exams dominate this decision: the NLE/NRE (Pakistan's own licensing exam, administered by PMDC), the DHA exam (Dubai Health Authority, UAE), the SMLE (Saudi Medical Licensing Exam, by the Saudi Commission for Health Specialties), and the MCCQE (Medical Council of Canada Qualifying Examination). Destination First

• NLE/NRE — required to practise medicine anywhere in Pakistan. Every MBBS graduate from a PMDC-recognised institution needs this regardless of any other plans.
• DHA — licenses you to practise in Dubai and, with DHA-equivalent recognition, much of the wider UAE healthcare system.
• SMLE — the entry exam for practising in Saudi Arabia under the Saudi Commission for Health Specialties framework.
• MCCQE — the qualifying route into Canadian medical licensure and, eventually, Canadian residency.

Unlike the NLE, none of the other three are optional extras — you sit the one that matches your destination, not all of them speculatively, unless you're deliberately keeping multiple countries open.

Format at a Glance

• NLE/NRE: computer-based MCQ exam covering the core MBBS clinical subjects, taken within Pakistan.
• DHA: computer-based MCQ exam, typically Prometric-administered, with content weighted toward general/family medicine plus your declared specialty.
• SMLE: computer-based MCQ exam built around a published blueprint, with Internal Medicine, Ob/Gyn, and Paediatrics together making up the bulk of the paper.
• MCCQE Part 1: computer-based clinical-knowledge exam with both MCQs and clinical decision-making (CDM) cases; Part 2 (OSCE) is Canada-only and comes later.

Clinical Style: What Actually Gets Tested

All four lean heavily on clinical-vignette-style questions rather than pure recall — a scenario, a set of findings, and a "what next" or "most likely diagnosis" question, rather than "define X." If you've built genuine clinical reasoning rather than memorised answer keys, that skill transfers across all four exams even though the content emphasis differs by country.

Who Should Choose Which

• Choose the NLE/NRE first, always — it's the baseline requirement to practise in Pakistan, and most graduates sit it before deciding on anything international.
• Choose DHA if the UAE, and Dubai specifically, is your target destination — it's a well-trodden path for Pakistani doctors with an established community of past candidates to learn from.
• Choose SMLE if Saudi Arabia is your goal — the exam blueprint is published, which makes structured prep more predictable than some alternatives.
• Choose MCCQE if Canada is the destination and you're prepared for a longer runway toward residency through CaRMS.

Some candidates sit more than one — most commonly NLE plus one international exam — to keep the Pakistan option open while pursuing licensure abroad. Sitting three or four in parallel is rare and usually not the efficient path; better to commit to the exam that matches your actual destination and prepare deeply for it.
