NLE Exam Pakistan: Complete PMDC Guide for MBBS Graduates

What Does NLE Stand For?
NLE stands for National Licensing Examination. It is the mandatory qualifying examination conducted by the Pakistan Medical and Dental Council (PMDC) that every MBBS graduate must pass before practising medicine in Pakistan. Without a passing NLE result, a doctor cannot obtain PMDC registration - and without PMDC registration, they cannot legally see patients, join a postgraduate programme, or apply for a Gulf health authority licence.
Why Was the NLE Introduced?
Before the NLE, fresh MBBS graduates were registered with PMDC automatically after completing their house job. The examination was introduced to standardise entry into the medical workforce, align Pakistan with international licensing frameworks such as the USMLE and PLAB, and ensure every practising doctor meets a minimum competency level regardless of which medical college they attended.
NLE Step 1 vs Step 2
The NLE is split into two steps:
- NLE Step 1 - Basic Sciences. Covers Anatomy, Physiology, Biochemistry, Pathology, Pharmacology, Microbiology, and Forensic Medicine. Candidates sit Step 1 after completing their MBBS degree but before starting the house job.
- NLE Step 2 - Clinical Competence. Tests applied clinical knowledge across Medicine, Surgery, Gynaecology and Obstetrics, Paediatrics, ENT, Ophthalmology, and Psychiatry. Candidates sit Step 2 after completing their one-year house job.
Both steps use a multiple-choice question (MCQ) format with single best answer (SBA) questions, making structured MCQ practice the most important preparation strategy.
Who Must Take the NLE?
- All MBBS graduates from PMDC-recognised Pakistani medical colleges
- Foreign-qualified MBBS holders seeking Pakistani PMDC registration
- Doctors re-registering under the new NLE framework
Eligibility Requirements
For Step 1: a completed MBBS degree from a PMDC-recognised institution. For Step 2: a completed house job of at least 12 months at an approved teaching hospital.
Exam Format and Pass Mark
Each step consists of 180-200 MCQs presented as clinical vignettes or basic science scenarios, each with five options and one best answer. There is no negative marking. PMDC sets the cut-off score internally and it may vary across sessions.
How to Register
- Log in to your account on the official PMDC portal.
- Select the NLE step and complete the online application.
- Upload required documents: MBBS certificate, transcripts, CNIC, and photographs.
- Pay the examination fee online.
- Download your roll number slip once approved.
How to Prepare Effectively
- Focus on high-yield subjects first. Pathology and Pharmacology dominate Step 1. Medicine and Surgery dominate Step 2.
- Practise clinical vignette MCQs. The NLE favours scenario-based questions over simple recall. Train yourself to reason through each case before selecting an answer.
- Work through past papers. NLE past papers reveal which topics PMDC returns to most and what a well-constructed vignette looks like.
- Review explanations thoroughly. Understanding why the correct answer is right - and why the wrong options are wrong - builds the pattern recognition that the NLE rewards.
- Simulate timed conditions. Regular timed practice ensures you can complete all questions without running out of time.
After Passing Both Steps
Full PMDC registration enables you to practise medicine in Pakistan, apply for FCPS or other postgraduate programmes, and use your registration as proof of qualification when applying to Gulf health authorities in the UAE, Saudi Arabia, Qatar, and beyond.
