FCPS vs MBBS: What's the Real Difference?

This is one of the most common questions from house officers and fresh MBBS graduates in Pakistan — and it's a slightly confused one, because MBBS and FCPS aren't actually alternatives to each other. They're sequential: FCPS is something you pursue after MBBS, not instead of it. Here's what each one actually is, and what the decision to pursue FCPS really involves.

Put simply: every FCPS holder in Pakistan is also an MBBS graduate. Not every MBBS graduate pursues FCPS. The real question isn't "FCPS or MBBS" — it's whether to stop at MBBS-level general practice or continue into specialist training.

What MBBS-Only Practice Looks Like

After completing MBBS and the mandatory one-year house job, a doctor can register with PMDC and practise as a general physician — working in general OPDs, emergency departments, primary care clinics, or as a medical officer in a hospital. This path requires no further postgraduate exam, and many doctors work productively this way, particularly early in their careers or while deciding on a specialty.

What it does not allow is independent practice as a named specialist (cardiologist, surgeon, gynaecologist, and so on) or a formal claim to specialist-level training — that requires a recognised postgraduate qualification such as FCPS, MS, MD, or an equivalent.

What Choosing FCPS Adds

Choosing to pursue FCPS on top of MBBS means committing to a formal, CPSP-accredited specialist training pathway:

• Structured residency training at a CPSP-accredited hospital in your chosen specialty, with a supervised logbook of cases and procedures.

Only after clearing both parts and completing accredited training does CPSP award the fellowship — at which point you can practise independently as a specialist consultant, take up academic posts, and (in most cases) have your qualification recognised for specialist-level practice in the Gulf, the UK, and several other countries.

Why Most Serious Career Doctors Still Pursue FCPS

MBBS-only practice is a legitimate, complete medical career on its own. But in Pakistan's job market, FCPS is the qualification that most consultant posts, most private-hospital specialist roles, and most academic medicine positions are built around. If the goal is to practise as a named specialist rather than a general physician — or to have a credential that travels internationally — FCPS (or an equivalent postgraduate qualification) is generally the route, not an optional extra.
